Patient - Doctor relationship
Dr Hofmeyr's relationship with her patients is based on trust, honesty and loyalty. If there are any questions or concerns, we encourage our clients to ask and discuss all so that we can ensure you have all the necessary information in order to make informed decisions regarding your health. Please do not hold back any questions or concerns, it is imperative that you understand all information in order for us to provide good healthcare. If you are one of our patients and have any non-emergency concerns/queries, please feel free to email us on [email protected] and we will get back to you as soon as feasible.
Confidentiality
We acknowledge our patient as the primary client and will always protect information we obtain from you to the best of our ability. In rooms we make use of electronic, audio and physical note-keeping in order to document our consultations thoroughly and share information discreetly with the rest of the healthcare and practice teams as necessary. When family and friends have concerns, it is the patient's responsibility to discuss the issues at hand with the healthcare team as we will not discuss case/client details without the patient present or via email or telephone conversation with anyone other than the patient herself.
Company during consultations
During consultations and examination, it is not possible to accommodate more than one supporting family member or partner in rooms due to space limitations. In pregnancy, and if arranged prior to the consultation, Dr Hofmeyr can allow a maximum of two people to accompany the patient during the ultrasound.
Dispute resolution
The process of dispute resolution in the practice is as follows:
1. If a dispute arises from, or is in any way connected to the terms and conditions of admission, services or treatment provided by Dr. Hofmeyr or any associate or locum with the practice (such as a claim for damages based on alleged medical malpractice) you are required to provide Dr. Hofmeyr with written notice of the nature and details of the dispute.
2. Dr. Hofmeyr may, at her sole discretion, refer any dispute notified in terms of the above or any such dispute which subsequently comes to its knowledge, for mediation. The referral must take place within 30 business days of receipt of such notice of dispute or from the date that the dispute comes to Dr. Hofmeyr’s knowledge, whichever is the later date.
3. The mediator shall be agreed between you and Dr. Hofmeyr, or failing agreement, shall be an accredited mediator appointed by the chairman for the time being of the bar council located in the jurisdiction governing Dr. Hofmeyr’s practice. You and Dr. Hofmeyr agree to abide by the mediation rules agreed, or failing agreement, the mediation rules of the Arbitration Foundation of Southern Africa (AFSA), in seeking to resolve the dispute.
4. If the dispute cannot be settled within 30 days (or such other period as agreed after the mediator has been appointed) or if Dr. Hofmeyr elects not to refer a dispute to mediation, then Dr. Hofmeyr may, at her sole discretion, refer the dispute to the chairman for the time being of the bar council located in the jurisdiction governing Dr. Hofmeyr’s practice to appoint an arbitrator who shall arbitrate the matter in accordance with the arbitration rules of AFSA but not under the auspices of AFSA.
5. Unless agreed otherwise, the place of arbitration shall be within the jurisdiction governing Dr. Hofmeyr’s practice.
6. The final (but not any interim) determination in the arbitration will be subject to appeal in terms of the Rules of AFSA, the appeal to be determined by an appellate panel of three persons appointed jointly by the parties to the dispute, or failing such joint appointment, appointed by AFSA.
