Dr Franelise Hofmeyr is a specialist obstetrician and gynaecologist attending to the concerns of women through all phases of life, with a special interest in pregnancy and the miracle of new life. Our practice strives to honour patients' rights at all times. We hold strong opinions regarding the sanctity of life, especially that of the unborn child. We believe women to be the pillars of society and we promote better health and opportunities for women of all shapes, sizes and shades. It is important that our clients are informed of the rules by which we practise as set out below.
When seeking medical advice or counsel from Dr Franelise Hofmeyr, in her consultation rooms or in the Christiaan Barnard Netcare Hospital, I agree to the following practice regulations:
1. By consulting Dr Hofmeyr for either elective or emergency medical care, I take full responsibility for the medical bill. As the self declared person responsible for my account (irrespective of whether I am the main member or a dependent on a medical aid) I accept that I am solely responsible to settle this account irrespective of my agreement with my personal medical aid/insurance.
2. I acknowledge that my medical aid membership is a personal agreement between the relevant scheme and myself and that if there is any delay or dispute regarding payment, I will settle the account personally within 30 days of services rendered.
3. If my account becomes overdue, I am aware that 2% interest will be charged per month and that legal steps may be taken with any additional costs incurred to be added to my account.
4. I undertake to notify the practice immediately, in writing, should any of my personal or medical aid information change.
5. I am aware that this practice charges private rates which is more than the Medical aid rates / Reference Price List (RPL).
6. I realize that I will receive additional bills for any blood tests, Papsmear, x-rays or procedures from the relevant laboratory or any other additional provider.
7. I am aware that Dr Hofmeyr reserves the right to bill specialist rates for all email and telephonic consultations as well as motivation letters and repeat scripts according to the practice's billing policy and that she will NOT correspond via sms or whats app regarding clinical questions or concerns.
8. Although Dr Hofmeyr will strive to be available for after-hours services or advice, I have been made aware that this practice will make use of locum doctors (including male colleagues) as per the discretion of Dr Hofmeyr. This includes deliveries and other emergencies.
9. Appointments will be fully charged for unless cancelled more than 24 hours in advance via telephone.
10. By agreeing to see Dr Hofmeyr I consent to the discreet disclosure of my personal and medical information to my Medical Aid as well as practice and hospital staff, mostly through the use of ICD10 codes, and I give consent to audio, electronic and physical record keeping of all consultations.
11. Dr F Hofmeyr encourages open communication between all parties if any unfortunate adverse events/complications occur and we encourage clients to contact the practice directly if any mismanagement (clinical or administrative) is perceived to allow us an opportunity to rectify or explain any perceived wrong-doing. If a client has any concerns regarding potential medical mismanagement, please note that this practice supports the utilization of an introductory and preliminary mediation meeting through a recognized mediator of the client's choice at no specific cost to the client.
12. Dr Hofmeyr is not a member of the Medical Protection Society (MPS) and in line with the practice's indemnity insurance as well as SASOG protocol, any complaint or dispute regarding the care provided by either Dr F Hofmeyr, her practice staff or locum associates shall firstly be addressed through a trial of formal mediation before litigation is pursued.
